For Newly Hired Teachers:
Use this form to set up the deposit for your paychecks! Be sure to attach a voided check before turning in to the district office.
This form needs to be filled out by your doctor after having a physical exam and TB test.
Fill out this form to enroll in TRS, or Teacher Retirement System, which is our pension program. You'll also name beneficiaries on this form.

Evaluation Information & Forms
This will outline the procedures and processes of our district evaluation plan.
Fill this form out before being observed by your evaluator. They will also discuss it with you before your observation.
Fill this form out after being observed by your evaluator. They will give you feedback following your observation.
Due October 1st. This is the action plan you fill out to describe your student growth goal and your method to achieve it. It will be discussed in your mid-year meeting.
Due February 1st. This is the form you fill out to describe outcome of your student growth goal, as well as your reflection on instructional practices and how you met your did not meet it.
